Care Manager – Case Manager (Hourly)
Program OverviewTo keep families together by providing safe, permanent housing and ongoing accessibility to evidence‑based trauma‑informed support services to Child Protection and Permanency families throughout the region.
Job OverviewThe Case Manager will provide comprehensive case management and supportive services to families experiencing homelessness or housing instability, helping them build stability and achieve long‑term self‑sufficiency. The role involves partnering with families to develop life skills, connecting them with community resources, facilitating groups, and maintaining accurate documentation to support positive outcomes. Evidence‑based practice (EBP) is emphasized.
Key Responsibilities- Provide support services and case management to families with a history of homelessness or housing instability.
- Assist parents in making a successful transition to self‑sufficient living and improved individual and family stability.
- Provide coaching and guidance on life skills such as home management, nutrition, budgeting, parenting, education/vocation training, and advocacy.
- Co‑facilitate groups for parents, children, and families.
- Link families to formal and informal community resources.
- Work collaboratively with KFT peers, DCP&P, and community service providers.
- Complete documentation consistent with program and agency requirements.
- Bachelor’s level practitioner or high school graduate with 3+ years of experience in direct case management to youth in a behavioral health agency or institutional setting.
- Valid driver’s license with an acceptable driving record.
- Ability to exercise independent thinking and good judgment in all circumstances.
- Ability to analyze work, set goals, develop plans, and utilize time effectively and efficiently.
- Starting Base Rate: $19.23 per hour.
- Sign‑on Bonus: $500.
- Schedule:
Day. - Bilingual Rate:
Additional $1.50 per hour for Spanish proficiency.
