Role Overview
The RN Hospice Visiting Nurse ensures quality and safe delivery of hospice nursing services to patients in the home or assisted living setting.
Responsibilities include:
- Developing and implementing the plan of care and making necessary revisions.
- Coordinating the plan of care and maintaining continuity with other health professionals.
- Supervising Licensed Practical Nurses and Home Health Aides and performing supervisory visits per regulatory guidelines.
- Providing education, supervision, and counseling to clients and their families regarding home nursing procedures and care needs.
- Ensuring accurate and timely electronic documentation and OASIS data entry.
Compensation
Annual salary with eligibility for weekly productivity bonuses.
Requirements
- Current RN license required.
- Hospice and/or Home Care experience preferred.
- New graduates will not be considered.
