BasicFunction
The Lead Autonomous Family Nurse Practitioner (FNP) serves as both a clinical leader and frontline provider. The role includes comprehensive medical services—assessment, diagnosis, prescription, treatment, and patient education—while leading a multidisciplinary team. The Lead FNP oversees and supports a team of APP, RN, and MAs, providing mentorship, clinical guidance, and workflow leadership. About 70% of the role is patient‑facing, with the remainder dedicated to leadership, team development, and operational improvement.
The clinic serves a diverse patient base, primarily adults with some all‑age care, and accepts all major insurance providers.
- Assess patient status through history, exam, and chart review, including mental health and psychosocial factors.
- Order and perform appropriate laboratory, diagnostic, and screening tests, consulting other clinicians as needed.
- Integrate data to determine diagnosis and therapeutic plan, identifying health risks.
- Develop and implement treatment plans, prescribing and dispensing medication and/or injections per guidelines and state law.
- Instruct patients and families on medications, treatments, health promotion, and community resources.
- Communicate case‑management information to other professionals and agencies.
- Participate in peer reviews, chart reviews, staff education, guideline development, and quality assurance activities.
- Provide day‑to‑day clinical leadership and foster a culture of collaboration, accountability, and continuous improvement.
- Serve as a clinical resource and mentor, guiding complex cases and clinical decision‑making.
- Optimize workflows, patient flow, and care coordination; establish and refine protocols, metrics, and pathways to improve outcomes and efficiency.
- Support onboarding, training, and skill development for clinical team members.
- Contribute to shaping clinic culture and supporting a mission‑driven, community‑focused primary‑care approach.
- Strong communication skills and a commitment to patient‑centered care.
- Comfort managing complex medical and psychosocial needs.
- Clinically versatile, capable of managing acute and chronic conditions across age groups.

- 5+ years as a Family Nurse Practitioner.
- 2+ years of clinical leadership experience, supervising clinical staff.
- Master of Science in Nursing.
- Completed Advanced Practitioner training and certified as a Family Practice Nurse Practitioner.
- Current state Registered Nurse license.
- Current state Advanced Registered Nurse Practitioner license with prescriptive authority.
