Chapters Health System is seeking an experienced RN Case Manager to assess patient and family needs, coordinate the Plan of Care with the IDT, and provide clinical, palliative, and supportive care in the home setting to keep participants in their home environment as long as possible.
The role requires an active RN license, at least 1 year of nursing experience (hospice or hospital preferred), and proficiency with EMR/EHR systems. Strong communication and mobility for home visits are essential.
