American Traveler is seeking an experienced RN Case Manager for a 13‑week acute inpatient care coordination position requiring an active MD or Compact RN license and at least 1 year of acute care nursing experience.

Requirements
  • Active MD or Compact RN license required — pending licensure not accepted
  • All certifications must be active at time of consideration
  • Associate’s Degree in Nursing (ADN) required
  • Minimum 1 year of experience in an acute medical/surgical, ED, or critical care nursing setting
  • Experience with electronic medical record (EMR) systems required
  • Knowledge of health care reimbursement systems including Medicare Advantage, Commercial payers, Medicaid, and CMS regulatory rules
  • Last 4 digits of SSN and date of birth required for consideration
Responsibilities
  • Acute inpatient hospital setting focused on care coordination and discharge planning
  • Responsibilities span the acute inpatient population including medical/surgical, ED, and critical care patients
  • Daytime shift: 8:30 AM – 5:00 PM, 8‑hour shifts, 40 hours per week
  • EMR:
    Epic (experience required)
  • Conducts initial face‑to‑face assessments for newly admitted patients within 24 hours to identify discharge needs
  • Collaborates with physicians, nursing staff, utilization review, and the broader care team to develop and document appropriate transition plans
  • Coordinates referrals to in‑network post‑acute vendors including Home Health, DME, IV Infusion, SNF Sub‑Acute, and Acute Rehab
  • Works with community providers and ambulatory case managers to facilitate safe discharges and prevent readmissions
  • Local candidates are accepted — local and travel rates are the same
  • Contingent workers may not exceed two years of total assignment time without a 90‑day break, per facility policy
