Responsibilities:

• Provide high-quality patient care to a diverse population of residents, including sub-acute rehabilitation and long-term care patients
• Develop and implement individualized care plans, ensuring patient-centered care is delivered
• Collaborate with interdisciplinary teams to ensure seamless communication and effective patient care
• Monitor and record patient vital signs, medication administration, and treatment plans
• Educate patients and families on treatment plans, self-care techniques, and disease management
• Participate in quality improvement initiatives to continuously improve patient outcomes and satisfaction

Requirements:

• Current Registered Nurse (RN) licensure in Maryland
• Registered Nurse (RN) experience preferred
