Job Title: Nurse Case Manager Department:Nursing

Supervisor: Director of Nursing FLSA Status:Exempt

Summary
The Nursing Case Manager is responsible for overseeing the health and medical care of individuals supported by Bello Machre. This role involves coordinating medical services, developing health plans, and ensuring compliance with all healthcare regulations. The ideal candidate is compassionate, detail-oriented, and committed to enhancing the well-being of individuals with developmental disabilities.

In addition to the requirements below, the employee must be at least 18 years of age, have a nurturing and caring demeanor, and have a genuine interest in supporting the independence of those we serve.

Education and/or Experience
Registered Nurse (RN) license required;
Bachelor of Science in Nursing (BSN) preferred. Minimum of 2 years of nursing experience, preferably in developmental disabilities or community health.

Trainings, Licenses, and Certifications
CPR and First Aid certification required.

The employee must have and maintain a valid Maryland driver’s license. They may not have more than three points on their license at any time and may not have any alcohol and/or drug related citations on their license.

Employees in this position perform tasks that may involve potential exposure to blood and other bodily fluids. All employees are expected to utilize universal precautions and follow all safety guidelines. Clinical Care Coordination

  • Develop, implement, monitor, and update individualized nursing plans of care based on assessed healthcare needs.
  • Coordinate routine, specialty, and emergency medical services for individuals receiving support.
  • Monitor changes in health status and collaborate with healthcare providers to ensure appropriate follow-up and continuity of care.
  • Communicate effectively with physicians, specialists, pharmacies, families, guardians, and interdisciplinary team members regarding medical needs and recommendations.

Health Oversight & Regulatory Compliance

  • Conduct nursing assessments, health monitoring, and wellness reviews in accordance with organizational policies and regulatory requirements.
  • Ensure compliance with all applicable federal, state, and local regulations, including Maryland Board of Nursing delegation requirements and Developmental Disabilities Administration standards.
