We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Autumn Lake Healthcare at Glen Burnie, a premier sub-acute rehabilitation and long-term care facility in Glen Burnie, Maryland. As a licensed practical nurse, you will play a vital role in providing high-quality patient care and contributing to the overall success of our nursing team.

As a Licensed Practical Nurse (LPN) at Autumn Lake Healthcare at Glen Burnie, you will be responsible for delivering individualized care to our residents, promoting independence, and enhancing their quality of life.

• Administering medications, treatments, and injections as prescribed by physicians
• Monitoring vital signs, reporting changes to nurses and physicians
• Providing wound care, ostomy care, and other treatments as needed
• Assisting with activities of daily living (ADLs) such as bathing, dressing, and grooming
• Documenting patient information accurately and thoroughly
• Collaborating with interdisciplinary teams to develop and implement individualized care plans
• Maintaining a clean and safe environment for residents and staff

• Current LPN license in the state of Maryland
• Previous experience in long-term care or sub-acute rehabilitation setting preferred
• Strong communication and teamwork skills
• Ability to work in a fast-paced environment and prioritize tasks effectively
