The AOT Care Coordination Certified Reentry Specialist is a vital member of Lead4

Life’s multidisciplinary Assisted Outpatient Treatment (AOT) Care Coordination Team. This position provides specialized support to adults with serious and persistent mental illness (SPMI) who are transitioning from incarceration or are actively involved in the criminal justice system. The Certified Reentry Specialist helps participants successfully reintegrate into their communities by connecting them with housing, identification documents, benefits, behavioral health services, employment resources, transportation, and other essential supports that promote long-term stability and recovery.

Working closely with AOT Care Coordinators, behavioral health providers, correctional partners, and community agencies, this position ensures participants receive coordinated, person-centered services that address legal, social, and economic barriers to treatment engagement and community success. The role is responsible for supporting compliance with individualized AOT Treatment Plans while promoting recovery, reducing recidivism, and improving health outcomes.

This position operates in accordance with Maryland Health-General Article.

• Provide comprehensive pre- and post-release reentry planning and coordination for AOT participants with current or recent correctional involvement, including coordination with Maryland correctional facilities, Parole and Probation agents, and the District Court to ensure continuity of AOT Treatment Plan services at the point of release.
• Assist AOT participants in obtaining critical identification and benefits documents required for community reintegration, including Maryland State  driver’s license, Social Security card, birth certificate, and proof of citizenship or immigration status, using established state and local document procurement pathways.
• Initiate and facilitate restoration of suspended or terminated public benefits — including Medicaid/Health Choice, SSI/SSDI, SNAP, and Maryland Energy Assistance — for participants exiting incarceration, coordinating with the AOT Care Coordination Case Manager to prevent gaps in psychiatric medication access and treatment funding.
• Collaborate with the AOT Care Coordination Case Manager and AOT Care Coordination Psychiatrist to incorporate reentry-specific goals and legal compliance requirements into each participant’s AOT Treatment Plan, ensuring court-ordered treatment obligations and community supervision conditions are mutually reinforcing and clearly communicated.
• Serve as the primary liaison between the AOT Care Coordination Team and community supervision officers (Parole and Probation, pretrial services, drug court staff), providing timely treatment compliance updates and facilitating coordinated responses to supervision violations or missed court appearances.
• Conduct individualized reentry risk and needs assessments using validated tools (e.g., LSI-R or equivalent) to identify criminogenic factors, mental health barriers, housing instability, and employment deficits that may impede successful community reintegration, and incorporate findings into the AOT Treatment Plan.
