Key Responsibilities
  • Provide high‑quality nursing care in patients’ homes in accordance with Family First Homecare policies, state Nurse Practice Acts, and all applicable regulations.
  • Maintain an updated clinical record for each patient, meeting all required documentation deadlines.
  • Evaluate the effectiveness of nursing services to patients and families on an ongoing basis.
  • Consult with physicians regarding changes to patient care plans and document all alterations and reports to the nurse supervisor.
  • Mentor and coach new employees during patient‑specific orientations.
  • Educate patients and caregivers on topics related to patient care needs.
  • Participate in the Quality Assurance, Performance Improvement (QAPI) program as requested.
  • Continuously strive to improve nursing care skills.
  • Attend and complete all mandatory training and in‑services assigned.
  • Submit required personnel file documentation in a timely manner.
  • Adhere to compliance, ethics expectations, patient and family confidentiality, and professional communication standards.
  • Perform other duties as assigned.
Role Qualifications
  • LPN/LVN nursing degree.
  • Unencumbered LPN/LVN license in the state of employment or an active multi‑state license through the Enhanced Nurse Licensure Compact.
  • At least 1 year of demonstrated experience.
  • Valid driver license with proof of insurance (when required).
  • Clear background screening.
  • Valid CPR card/certificate with in‑person instructor‑led hands‑on skills check.
  • Clear Hepatitis

    B and TB screening (state requirements apply).
Preferred Qualifications
  • State‑specific knowledge and certifications (detailed by state below).
State Specific Requirements
  • Florida
    • Valid driver license with proof of insurance not required. Staff prohibited from driving patients.
    • Hepatitis

      B screening not a prerequisite for hire; completed during onboarding.
  • Illinois
  • Pennsylvania
    • Additional background screening for child abuse and FBI check.
    • Must complete TB screening with 2‑step PPD or provide proof of 2‑step PPD administration within the last year.
  • Texas
  • Other states: check applicable regulations.
Key Competencies
  • Ability to read and communicate in English, both verbally and in writing.
Working Conditions / Equipment Use
  • Work performed in patient homes; no substantial exposure to adverse environmental conditions.
  • Training may occur in an office setting.
  • Must be able to lift up to 35 pounds unassisted.
  • Frequent use of office equipment including telephone, computer, and printer.
