- Provide comprehensive nursing services within the scope of practice.
- Maintain updated clinical records and meet documentation deadlines.
- Evaluate the effectiveness of nursing services and consult with physicians concerning care plan alterations.
- Report all changes in care to the nurse supervisor.
- Mentor or coach new employees during patient‑specific orientations.
- Educate patients and caregivers on care needs.
- Participate in FFHC Quality Assurance and Performance Improvement (QAPI).
- Attend mandatory trainings and in‑services as assigned.
- Submit required personnel documentation timely.
- Adhere to compliance, ethics, confidentiality, and professional communication standards.
- Perform other duties as assigned.
- LPN/LVN nursing degree.
- Unencumbered LPN/LVN license in the state of employment or Multi‑State license via the Enhanced Nurse Licensure Compact.
- At least 1 year of demonstrated experience.
- Valid driver’s license with proof of insurance (if required).
- Clear background screening.
- Knowledge of state Nurse Practice Acts.
- Valid and current CPR card with in‑person instructor‑led skills check.
- Clear Hepatitis
B and TB screening (where applicable).
- Additional experience with pediatric homecare.
- Experience with 1:1 nurse‑to‑patient ratio.
- Florida:
No requirement of driver’s license;
HepB screening completed during onboarding. - Illinois:
No driver’s license requirement. - Pennsylvania:
Additional background screening for child abuse and FBI check; TB screening with 2‑step PPD. - Texas:
No additional state‑specific requirements.
- Ability to read and communicate in English both verbally and in writing.
Work is performed in patients’ homes with no substantial environmental hazards. Training may occur in an office setting. Must be able to lift up to 35pounds without an assisted lifting device. Frequent use of office machines such as telephone, computer, and printer.

Pay PhilosophyThe starting salary range reflects skills, experience, education, certifications, and location, and is designed to compete with the national market. Compensation may include bonus opportunities as outlined in the compensation plan.
Key DetailsThis role requires travel to patients’ homes. The schedule may include varying shift lengths (8, 10, or 12hours) and may require availability on any day of the week, Sunday through Saturday.
