Job Summary
Family First Homecare is seeking a licensed Practical Nurse (LPN) to provide in‑home nursing care to pediatric patients. The role involves delivering comprehensive nursing services while complying with company policies and state and federal regulations.
Key Responsibilities- Accountable for providing care that is based on licensure/certification within the scope of practice.
- Maintain updated clinical records for each patient at all times, meeting required documentation deadlines.
- Evaluate the effectiveness of nursing services to patients and families on an ongoing basis.
- Consult with physicians regarding alterations to patient care plans and report changes to the nurse supervisor.
- Support FFHC’s mission by mentoring and/or coaching new employees during patient‑specific orientations.
- Educate patients and caregivers on topics related to patient care needs.
- Participate in FFHC Quality Assurance, Performance Improvement (QAPI) program as requested.
- Continuously improve nursing care skills.
- Attend and complete all mandatory training and in‑services as assigned.
- Submit required personnel file documentation in a timely manner.
- Adhere to FFHC compliance and ethics expectations.
- Maintain patient and family confidentiality.
- Maintain professional, supportive, and responsive interpersonal communication skills.
- Perform other duties as assigned.
- Education and Experience
- LPN/LVN nursing degree.
- Unencumbered LPN/LVN license in the state of employment or multi‑state license via the Enhanced Nurse Licensure Compact where recognized.
- At least 1 year of demonstrated experience.
- Valid driver’s license with proof of insurance in applicable states (except where state requirements differ).
- Clear background screening.
- Working knowledge of Nurse Practice Acts.
- Valid and current CPR card/certificate with instructor‑led hands‑on skills check.
- Clear Hepatitis‑B screening in applicable states.
- Clear TB screening in applicable states.
- Preferred / State Specific Requirements
- Florida:
No driver’s license requirement; staff prohibited from driving patients;
Hepatitis‑B screening completed during onboarding. - Illinois:
Same as Florida. - Pennsylvania:
Additional background screening for child abuse and FBI check; must complete TB screening with 2‑step PPD or provide proof of 2‑step PPD within the last year. - Texas:
No additional state‑specific requirements.

Work is performed in the patient’s home and is not substantially exposed to adverse environmental conditions. Training may be conducted in an office setting. The candidate must be able to lift up to 35 pounds without an assistance device and use office machines such as telephone, computer, and printer.
