RN Field Hospice - Float Nurse

Location:

Glen Carbon, IL.

Mon‑Fri 2:00pm – 10:30pm, Op;
Paid travel time.

As a Hospice Field RN, you provide skilled nursing care to terminally ill adults and children, delivering symptom management, pain control, counseling, and bereavement services in home or skilled nursing facility settings.

Evaluates patients and furnishes services requiring specialized nursing procedures and instructions to assist the client in learning self‑care techniques, coordinating all aspects of care when assigned as case manager.

• Assess patient preferences and barriers to involvement in care, including values, emotional, spiritual, cultural, and population‑specific needs.
• Develop, implement, and document individualized care plans with defined goals in collaboration with the interprofessional team and patient, family, or caregiver.
• Proactively plan and ensure communication of the care plan across the continuum of care.
• Promote respect, equity and empathy through care delivery and support for emotional, spiritual, and cultural preferences.
• Practice collaborative problem‑solving, service recovery, and advocacy for patient family‑centered continuity of care.
• Integrate data from the interprofessional team and apply critical thinking to implement care safely and timely.
• Evaluate changes in patient condition, inform and collaborate with family and/or caregivers, and update the plan of care in the EMR.
• Ensure evidence‑based practice and quality patient outcomes by evaluating current nursing care.

• Bachelor's Degree in Nursing or Home Health.
• 2–5 years of hospice, home health, or skilled nursing experience.
• Strong assessment, IV, and symptom management skills.

• Education:
  
  Nursing Diploma/Associate’s Degree in Nursing.
• Experience:
  
  < 2 years.
• No prior supervisory experience required.
• Valid Driver’s License.
• BLS (Basic Life Support) required.

• Valid RN license.
