Job OverviewLicensed Practical Nurse (LPN) – Pediatric G‑tube Home Care (5:45a.m. – 4:30p.m.)
Key Responsibilities- Provide comprehensive nursing services in accordance with Family First Homecare policies, state Nurse Practice Acts, and applicable regulations.
- Maintain updated clinical records and complete documentation by required deadlines.
- Evaluate nursing service effectiveness and consult with physicians regarding changes to patient care plans.
- Report all changes in care to the nurse supervisor and coordinate with team members.
- Mentor new staff during patient‑specific orientations.
- Educate patients and caregivers on care topics.
- Participate in Quality Assurance and Performance Improvement (QAPI) activities as requested.
- Continuously improve nursing care skills and attend all mandatory training/in‑services.
- Submit required personnel file documentation promptly.
- Adhere to compliance, ethics, and confidentiality expectations.
- LPN/LVN degree and unencumbered license in the state of employment (or multi‑state license via the Enhanced Nurse Licensure Compact).
- Minimum 1 year of demonstrated experience.
- Valid driver’s license with proof of insurance in applicable states.
- Clear background screening.
- Working knowledge of Nurse Practice Acts.
- Current CPR card/certificate with in‑person, instructor‑led hands‑on skills check.
- Clear Hepatitis B and TB screening, as required by state.
- Experience with pediatric patients and G‑tube care.
- Knowledge of state‑specific additional requirements (e.g., Pennsylvania child‑abuse, FBI check).
- Strong communication skills in English.

Work is performed in patients’ homes with minimal environmental hazards. Training may occur in an office setting. The nurse may lift up to 35pounds without an assisted lifting device and will use office equipment such as telephone, computer, and printer.
