Job DescriptionHome health registered nurse case managers lead and collaborate to manage and deliver patient care plans in Belleville and surrounding areas. This full-time position offers a total bonus potential of $15,000, including a $10,000 first-year success bonus and a $5,000 sign-on bonus paid within the first 18 months. To receive these bonuses, nurses must be in good standing, meet performance expectations, and remain in the role and status for which they were hired.
Responsibilities- Develop and implement individualized care plans for patients.
- Travel to patient locations, driving up to ~20% of the day.
- Maintain critical charting deadlines and documentation accuracy.
- Self-manage time and schedule using tablets, mobile phones, and EMR software.

- RN license in the state where you work.
- Graduate from an approved practical nursing school.
- At least one year of RN experience and clinical competence to deliver quality patient care.
- Current driver’s license and ability to spend ~20% of the day driving to and from patient locations.

- Ability to self-manage time and schedule.
- Demonstrated experience with tablets, mobile phones, and EMR software.
