Role OverviewA registered nurse in this home care role provides 1:1 patient care to an underserved community, supporting patients in their homes while promoting work-life balance and flexible scheduling. The nurse builds meaningful relationships, assesses patient needs, develops and implements individualized care plans, and collaborates with interdisciplinary teams to ensure quality outcomes. Responsibilities include evaluating patient conditions, respecting diverse cultural and emotional needs, and updating care plans accordingly.
The position is safety-sensitive, requiring alertness and adherence to safety protocols.
- Educational requirements:
Nursing Diploma/Associate's Degree in Nursing - Minimum 2 years of nursing experience; preferred Bachelor's Degree in Nursing or Home Health experience
- Valid RN license and driver's license
- Strong assessment, communication, and critical thinking skills
- Ability to work independently and provide comprehensive patient care
- Assess patient preferences, barriers, and needs
- Develop, implement, and document personalized care plans
- Coordinate care with team members, patients, and families
- Monitor patient progress, evaluate changes, and adjust plans accordingly
- Promote respectful and culturally sensitive care
