Epicured Social Care Navigator

Epicured is hiring Social Care Navigators to support New York's Medicaid Section 1115 Waiver program. In this role, you'll engage Medicaid members, assess eligibility for Health-Related Social Needs (HRSN) services, and coordinate referrals through Epicured and community partners.

This role, reporting to the VP of Member Experience, is ideal for someone who is organized, compassionate, tech-savvy, and comfortable working in a high-volume virtual contact center environment.

• Manage inbound and outbound member communications via phone and email, leveraging Five9 and various partner technology platforms
• Conduct outreach to screen potential members, assess eligibility, and navigate members to service providers based on identified health-related social needs
• Track and follow up on referrals to ensure service delivery and satisfaction
• Maintain accurate documentation in program platforms and internal systems
• Support members with program questions
• Comply with Medicaid, HIPAA, and NYS DOH requirements

Pay: $21–$23/hour, Non-exempt, based on experience
