Registered Nurse; RN - Home Health

Position Overview

Graham Healthcare Group is hiring a Registered Nurse (RN) for Home Health in Glendale Heights, Illinois.

• Deliver and manage patient care plans in home health settings across Glen Ellyn and surrounding areas.
• Act as a case manager, coordinating care with other team members.
• Maintain accurate and timely charting to meet critical deadlines.
• Utilize tablets, mobile phones, and EMR software for documentation and communication.
• Drive to patient locations, spending approximately 20% of the workday on travel.

• Valid RN license in the state of practice.
• Graduate of an approved school of practical nursing.
• Minimum one year of RN experience with clinical competence in patient care.
• Current driver’s license and ability to travel to patient locations.
• Strong time‑management and scheduling skills.
• Experience with tablets, mobile phones, and EMR software.

Position:
Full‑time, 40 hours per week, 8‑hour shifts, ongoing.

Compensation:
Competitive pay per visit, 4‑month orientation pay guarantee, monthly clinical outcome bonuses after one year, first‑year success bonus of up to $10,000.
