As an Acute Clinical Staff Pharmacist
, you will be responsible for the delivery of pharmaceutical care to patients using the medication use process such as selecting, prescribing, dispensing, administration and monitoring of medications. You will perform comprehensive assessments and analysis of drug therapy in collaboration with patient care team members. You will be accountable for ongoing evaluation and documentation of all components of pharmaceutical care delivered to patients.

You will oversee the activities of the pharmacy technicians, coordinating workflow and checking all medications prior to being dispensed.

The schedule for this position will be four, ten‑hour shifts 12PM - 10:30PM, including every other weekend.

Position

Summary

This position is responsible for the delivery of pharmaceutical care to patients using the medication use process. It is accountable for documentation of all components of pharmaceutical care they deliver to patients. The pharmacist oversees the activities of pharmacy technicians and interns, coordinating workflow and approving medications prior to being dispensed. The pharmacist must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients being served, assess data reflective of the patient's status, interpret the appropriate information needed to identify each patient's requirements and provide pharmaceutical care based on those needs.

The pharmacist must adhere to local, state, and federal laws regarding the profession of pharmacy practice.

Core Functions
  • Processes medication orders and evaluates their appropriateness and completeness with respect to patient data including age, height, weight, diagnosis, laboratory values, and other pertinent patient information.
  • Utilizes clinical pharmacy skills to assess patient medication regimens and formulates recommendations to ensure optimal safety, efficacy, and cost‑effectiveness for all prescribed medication therapies. Contacts the prescriber with suggestions for improvement and resolves problems with medication orders as needed. Serves as a resource for patients and the health care team. Provides requested medication information in a timely manner.
  • Complies with directives of the Pharmacy and Therapeutics Committee/Clinical Consensus Group. Documents activities related to patient care in the electronic medical record and other electronic databases as applicable (e.g., pharmacist to pharmacist/provider communication, adverse drug event reporting, and clinical interventions).
