Job Description

Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.

Deliver expert pharmacy care as a home infusion clinical pharmacist, specializing in preparing, dispensing, and monitoring complex intravenous (IV) and subcutaneous medications. This role serves as a vital link within a multidisciplinary team, ensuring safe sterile compounding, precise medication adjustments, and thorough education for patients and caregivers.

Mesa, AZ
Job Type: Contract (8-12 weeks)

Schedule:

• Monday to Friday, 8:30 AM – 5:00 PM
• May include on-call rotation
  Start: ASAP

Key Responsibilities:

• Prepare specialty pharmaceuticals and IV solutions in compliance with state and federal sterile compounding regulations.
• Monitor patients by assessing lab values, calculating pharmacokinetic dosing, and managing Total Parenteral Nutrition (TPN).
• Provide detailed counseling and education on specialty medications, infusion pump use, sterile techniques, and catheter care to patients and caregivers.
• Coordinate care with prescribing physicians, home health nurses, and internal pharmacy teams to optimize therapy and reduce hospital readmissions.
• Ensure strict adherence to regulatory standards including State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, and OSHA.
• Manage inventory effectively, including refrigerated medications, and prepare timely shipments to patient homes.

Desired

Qualifications:

• 1 to 3 years of infusion specialty pharmacy experience or hospital pharmacy IV room experience.
• IVIg experience highly desirable.
• Strong understanding of specialty therapy payer requirements, reimbursement processes, and contract compliance.
• Proven competence with Care Tend pharmacy systems.
• Current, active pharmacist licenses in Arizona and Texas in good standing.
