PHYSICAL THERAPIST – FULL TIME BELLA VITA HEALTH & REHABILITATION – GLENDALE, AZ

Bella Vita Health & Rehabilitation in Glendale, AZ, is a post‑acute care facility that provides long‑term care, short‑term care, and geriatric outpatient services. We are looking for a Full‑Time Physical Therapist to join our in‑house therapy team. You will work in a supportive environment that values excellence, leadership, and professional growth.

Full Time – Flexible schedule with work‑life balance.

Pay – $42–$50 per hour based on experience.

• Screen, evaluate, and develop appropriate plans of care for patients with cognitive and/or ADL functional disorders following regulatory and clinical standards.
• Utilize standardized assessments and evidence‑based practice to guide clinical interventions.
• Provide rehabilitative, skilled, and medically necessary treatment interventions, including ADL retraining, therapeutic activities, exercises, modalities, and the use of supportive and assistive devices.
• Asses patient progress and adjust treatments accordingly in the clinical record.
• Evaluate treatment outcomes for effectiveness and modify services or make referrals as needed.
• Provide consultation and counseling to patients, families, caregivers, and other service providers about physical disorders.
• Communicate effectively with supervisors and health‑team members regarding patient progress, barriers, and treatment plans.
• Prepare comprehensive discharge summaries of services provided per regulatory requirements.

• Licensed Occupational Therapist (licensed PT preferred).
