This is a deeply meaningful, patient- and family-centered role embedded within a pediatric hematology and oncology ambulatory care setting. As the sole social worker on a close-knit team, you will walk alongside families during some of the most challenging seasons of their lives — connecting them with foundations, financial resources, and community support to ease the everyday burdens that don't stop when treatment does.

Many pediatric patients are treated over a span of 3–5 years and continue to be seen long after active treatment ends, making long-term relationship-building and consistent follow-up a cornerstone of this role. You'll collaborate with a multidisciplinary team of providers, nurse navigators, nurses, and child life specialists. This is a role where your work directly gives families more time to simply be present with their loved one.

Key Responsibilities Provide psychosocial support and care coordination for hematology and oncology patients and families

Connect patients and families with foundations and financial assistance programs to address ongoing expenses

Manage a daily caseload of approximately 4–5 families, with up to 3–4 inpatient and up to 8 outpatient clinic appointments

Conduct long-term follow-up with patients, including those who have completed active treatment

Collaborate with a multidisciplinary team including patient navigators and a child life specialist

Preferred Qualifications Experience in pediatric and/or oncology social work strongly preferred

Comfort working independently while remaining connected to a supportive leadership team

POSITION SUMMARY This position develops, coordinates and provides social work services to patients and families who are faced with social, emotional and situational stressors precipitated by illness, injury, and/or disability. The goal is to aid in adaptation and empower the patient and the family to participate to the fullest of their abilities in the discharge planning process. This position provides developmentally appropriate care for the population that it serves which includes planning for the safe discharge and continuity of care, the ability to recognize and plan for the unique needs of all ages as well as the physically disabled, mentally ill, chronically ill and terminally ill patient.
