Overview
RN Case Manager – Travel/Local – Glendale, AZ. We’re hiring an experienced RN Case Manager for a hospital-based role in Glendale, Arizona. This position focuses heavily on discharge planning and coordinating post-acute care to ensure safe, efficient patient transitions.
Responsibilities- Conduct initial case management assessments
- Develop and coordinate discharge (DC) plans with patients, families, and care teams
- Arrange post-acute services based on patient preferences and needs
- Collaborate with physicians, nurses, and interdisciplinary teams
- Ensure compliance with hospital policies and regulatory standards
- Maintain accurate and timely documentation
- RN License – State of Arizona OR Licensed Master’s Level Social Worker in AZ
- Minimum 5 years Case Management experience in an acute care hospital
- Strong experience with discharge planning and post-acute placement
- Excellent communication, coordination, and problem-solving skills
