This position assesses, plans, implements, evaluates, and documents nursing care of patients in accordance with organizational policies and professional nursing practice standards. The role is accountable for the quality of nursing services delivered by the individual and staff under their direction. Specialized knowledge and judgment are required to assess data, plan, provide, and evaluate care appropriate to the physical and developmental age of assigned patients.
CoreFunctions
- Assesses patient physical, psychological, social, spiritual, educational, developmental, cultural, and discharge planning needs. Reviews patient history and physical with patient/family and ensures timely completion. Reviews diagnostics and laboratory data; reports abnormal results to physicians and other caregivers. Completes assessments and reassessments per policy.
- Formulates a plan of care, including the discharge plan, using assessment data and patient/family health team input. Initiates a plan based on patient-specific needs and the medical plan of care. Goals are realistic, measurable, and developed with patient/family. Considers physical, cultural, psychosocial, spiritual, age-specific, and educational needs. Reviews and revises the plan as patient needs change.
- Implements the plan of care through direct patient care, coordination, delegation, and supervision of the health care team. Provides care per physician orders and nursing plan, complying with policies, standards of care, and regulatory requirements. Delegates appropriately and supervises other licensed nurses and personnel. Communicates patient status accurately and completely to other caregivers.
- Evaluates the patient’s and family’s response to care and teaching, revising the plan of care as needed. Assesses patient progress toward goals and expected outcomes with the team, evaluating teaching effectiveness.
- Documents assessment, planning, implementation, and evaluation in the patient record. Documentation is legible, timely, and follows policy, reflecting objective/subjective data, nursing interventions, and patient response. Notes physician orders accurately and timely.
- Provides evidence-based care and may participate in research activities. Participates in unit or facility shared leadership and in education, role development, and orientation of personnel, patients, families, and visitors. Supports growth of others through precepting and mentoring when appropriate.
- Contributes to society through activities that promote excellent patient outcomes via timely, effective, efficient, equitable, and safe care. Participates in improving national nursing quality indicators and outcomes; may join professional organizations.
- May supervise staff and workflow of the department. Interacts with all levels of staff across departments, physicians, patients, families, and external contacts such as other health care institutions, community providers, and agencies concerning patient care. Works with clergy for spiritual support and synthesizes data from multiple sources to support patient and family responses to health status changes.
Must possess a current, valid RN license in the state of practice, or an equivalent temporary/interim or compact RN license. BLS certification is required. Additional certification or continuing education may be required based on the area of practice. Experience in the clinical area for which the role applies is desired. Must maintain clinical performance competencies appropriate to the area, demonstrated through annual validations.
For Registry and Travel acute care positions, a minimum of one year of experience in an acute care hospital setting and at least 12 months as a Registered Nurse in the specialty area are required.
Bachelor’s degree preferred. Professional certification preferred.
