Canyon-Home-Care-and-Hospice-1 is seeking a Registered Nurse to provide skilled nursing care in patients’ homes in the West Valley area. You will perform initial evaluations, develop and revise the plan of care, and collaborate with the IDT to meet patient needs.
You will regularly reassess nursing needs, document care, and coordinate with physicians, therapists, patients, and families. OASIS assessments and on-call duties are part of the role.
