Canyon Home Care and Hospice is seeking a Registered Nurse Case Manager to provide intermittent skilled nursing care in patients' homes in the Laveen/Phoenix area. You will perform, with physician orders, initial evaluation, develop and adjust the plan of care, and coordinate with the IDT to ensure safe, effective care.
Current RN license in the state and 1 year nursing experience preferred; you will supervise LPN/LVN and HHA/CNA per schedule and maintain timely documentation and care
