Job Description
The Registered Nurse Home Health Case Manager plays a vital role in delivering comprehensive nursing care to patients in their home environments. This position involves assessing patient needs, managing medications, educating patients and caregivers, and coordinating multidisciplinary care within a team-based caseload. The nurse will utilize an electronic health record system to document care and communicate effectively with all stakeholders to ensure optimal patient outcomes.
Responsibilities- Conduct thorough patient assessments during in-home visits
- Manage and monitor patient medication regimens
- Educate patients and caregivers on disease management and health maintenance
- Coordinate care among healthcare providers, specialists, and community resources
- Maintain accurate and timely documentation using electronic health record systems
- Communicate effectively with patients, families, and team members
- Collaborate with a multidisciplinary, team-based caseload
- Minimum of 1+ years experience in nursing
- Active Registered Nurse (RN) license
- Proficiency in patient assessment and care coordination
- Experience with electronic health record (EHR) systems
- Background in home health care and medication management
- Excellent communication, time management, and problem-solving skills
