We’re hiring an experienced RN Case Manager for a hospital-based role in Glendale, Arizona
. This position focuses heavily on discharge planning and coordinating post-acute care to ensure safe, efficient patient transitions.
Conduct initial case management assessments
Develop and coordinate discharge (DC) plans with patients, families, and care teams
Arrange post-acute services based on patient preferences and needs
Collaborate with physicians, nurses, and interdisciplinary teams
Ensure compliance with hospital policies and regulatory standards
Maintain accurate and timely documentation
RequirementsRN License – State of Arizona
OR Licensed Master’s Level Social Worker in AZ
Minimum 5 years Case Management experience in an acute care hospital
Strong experience with discharge planning and post-acute placement
Excellent communication, coordination, and problem-solving skills
