Job Description

Position Summary:

The SMI (Seriously Mentally Ill) Services Case Manager is responsible for providing comprehensive case management services to adults diagnosed with serious mental illness. The Case Manager works as part of a multidisciplinary team to support clients in achieving stability, independence, and improved quality of life. This position involves care coordination, advocacy, service planning, crisis intervention, and collaboration with community partners to ensure continuity of care and access to resources.

Key Responsibilities:

  • Conduct assessments and treatment plan to identify client needs, strengths, goals, and barriers.
  • Develop individualized service plans in collaboration with clients, family members, and treatment team.
  • Coordinate and monitor service delivery, including referrals to mental health, medical, housing, employment, substance use, and social services.
  • Maintain regular contact with clients through home visits, phone calls, and meetings.
  • Provide psychoeducation, supportive counseling, and crisis intervention as needed.
  • Advocate for clients to ensure access to entitlements, benefits, and appropriate services.
  • Collaborate with psychiatrists, therapists, housing providers, law enforcement, and other stakeholders to support client care.
  • Maintain accurate and timely documentation in compliance with agency policies, state, and federal regulations.
  • Participate in team meetings, trainings, and case consultations.

Qualifications:

  • Experience:

    Minimum of 1-2 years of experience working with individuals with serious mental illness or in a related social services field.
  • Licensure/Certification: CPR and Crisis Response training certificate
  • Other Requirements:
    • Valid driver’s license and reliable transportation.
    • Ability to pass background checks and required screenings.
    • Strong interpersonal, organizational, and communication skills.
    • Proficient in using electronic health records and Microsoft Office Suite.

Preferred Qualifications:

  • Familiarity with community mental health systems, Medicaid/insurance programs, and recovery-oriented approaches.
  • Bilingual in English and Spanish or other languages is a plus.
  • Experience with crisis intervention or mobile outreach services.

Work Environment & Physical Demands:

  • Work is performed in both office and field settings, including client homes and community locations.
  • Must be able to drive to various locations and transport clients if necessary.
