Community Health Worker (Enhanced Care Management)
This is a hybrid position, and you have the option to elect into a 9/80 flex work schedule. Candidates are required to be in the office one day per week, with field work and flexibility to travel within Inglewood and Glendale areas as needed.
The pay range for this position is between $19.87-$22.85 per hour.

As a Community Health Worker (CHW), you will draw on your valuable lived experience to promote understanding of members’ perspectives and preferences. You will address chronic conditions, preventive health needs, and health-related social needs. Working alongside Lead Care Managers (LCMs) and paraprofessionals, you will foster client engagement, advocate for members, provide case management support, and triage clients to appropriate levels of care and community resources.
PrimaryDuties
- Engage eligible ECM Members utilizing various modalities including in-person and tele-health.
- Build strong relationships with health plan members to educate them about the benefits and eligibility criteria for ECM as means of increasing ECM participation.
- Outreach to community members through various channels, including community events, workshops and outreach programs to raise awareness.
- Accompany ECM Member to office visits, as needed, and in the most easily accessible setting, within MCP guidelines. Advocate on the member’s behalf with health care professionals.
- Promote health promotion and self-management training that addresses barriers to physical and mental health care, such as providing information or instruction on health topics including but not limited to distributing health promotion materials. Monitor treatment adherence (including medication) and communicate adherence concerns to LCM and other treatment providers.
- Provide health navigation services, including training, information, referrals, and support members in accessing health care, understand the health care delivery system, or engage in their own care.
- Arrange transportation to medical appointment using MCP medical transportation system. Call Member to facilitate visit with LCM and other care providers.
- Assist with linkage and connect ECM Member to other social services and supports the member may need.
- Use motivational interviewing, trauma-informed care, and harm-reduction approaches. Monitors and reports member’s behavior, schedule, medications and activities and provides relevant feedback of any behavioral deviations from recovery to team and relevant stakeholders to support timely responses of care.
- Maintains documentation of all groups and individual member service activities; ensures compliance with Didi Hirsch policies and procedures, contracts and other regulatory agencies.
- Develops and maintains CHW skills through seminars, workshops or other forms of training/education related to assigned duties.
- Consults with management to support informed decision-making care prior to taking actions related to programming and services.
- Attends and actively participates in all team and staff meetings, community meetings and supervision/consultation.
- Meet agency client service expectations.

- High school diploma or GED required.
- Certified as Community Health Worker (CHW) through the completion of a qualifying training program and/or have lived experience which could include navigation of the mental health system and/or substance use treatment system as a consumer.
