This unique clinic-based telehealth role combines the convenience of virtual care delivery with the support of an on-site, collaborative team environment ideal for providers passionate about innovative, team-based primary care.

• Clinic-based on-site at Banner Health Center Plus, 7701 W. Aspera Blvd., Glendale, AZ 85308
• Provide telehealth visits to established patients and manage clinical inbox for care coordination
• Flexible scheduling: 4x10s or 5x8s shifts
• Medical Assistants, Front Office, and leadership support
• Support of an integrated system including specialists, urgent care, and more!

Our Glendale Banner Health Center Plus, located off the Loop 101 and 75th Avenue, offers primary care, pediatrics, specialty medicine, imaging, lab, and pharmacy in one convenient location. This state-of-the-art facility provides a collaborative, team-based environment with comprehensive resources and reduced patient wait times everything you need to deliver excellent care.

• DEA with prescriptive authority required
• FNP designation with AZ license eligibility
• Minimum (1) year of experience as a FNP in a primary care setting
• Patient-focused, friendly, and willing to collaborate in a team environment
