If you meet these qualifications, we would love to meet you:
- MSW - Social Worker
- Master's degree required
- Registered Nurse
- Associate's degree, preferred
- Registered Nurse license required in state(s) practicing
- Previous experience in hospice, home health, general medical surgical or patient population specific (i.e.: geriatrics, pediatric, etc.) nursing.
- LVN
- Associate's degree, preferred
- Current and valid Licensed Vocational Nurse license in state(s) where practicing required
- Chaplain
- Bachelor's Degree from a Council for Higher Education accredited college, university or divinity school is required; a graduate-level master's degree from a similarly accredited college, university or theological school preferred.
- Ordained, credentialed, or commissioned according to the practices of a legitimate and recognized religious organization/faith tradition.
As a Patient Family Liaison Clinical, you will:
- Collaborate with Admissions Department and Marketing staff to manage patient family information and sign-on visits.
- Communicate appropriately with providers, attending physicians, other appropriate hospice staff members and other agencies as needed to ensure excellent provider relations.
- Help the patient and family understand, accept, and follow medical recommendations. Provide services planned to assist the patient and family in achieving the optimum social, spiritual and heath care adjustments within their capacity.
- Partner with staff on in-services, and education related to advanced care planning.
- Maintain confidentiality of sensitive information per Health Insurance Portability & Accountability Act (HIPAA) regulations.
