• Travel to patients’ homes to administer services and help maintain independence.
• Administer at‑home IVs, change dressings, clean wounds, and perform in‑depth assessments.
• Oversee patient treatment plans and update doctors and the agency on patient status via Synergy software.
• Perform initial comprehensive assessments; document and upload to the agency portal.
• Obtain and document physician orders.
• Initiate and routinely re‑evaluate and revise plans of care in consultation with the assigned Clinical Team Manager.
• Coordinate care with the Clinical Team Manager, physician, and other home‑care team members, informing them of significant changes.
• Perform re‑certifications, resumption of care, transfers, and discharges as requested, completing related documentation.
• Assess need for services of other team members (PT, OT, ST, MSW, HHA).
• Perform Home Health Aide supervisory visits per regulatory requirements.
• Demonstrate knowledge of regulatory requirements.

• Current CA RN License (Required)
• BLS from the American Heart Association and/or the American Red Cross (Required)
• NSO Nursing License Insurance (Required)
• IV/Phlebotomy Certified (Required)
• Comprehensive knowledge of OASIS documentation (Preferred)

Availability: 7 days a week, with flexibility. Must work at least two‑three days each week (or ten productivity units), one weekend a month, and two holidays per year (one winter and one summer).

Location:

City close to you with a group of patients for that day.
Travel:
Home Health RN (1 year required experience).
