We are seeking a clinically experienced LVN to support nursing staff in a Home Care setting by performing patient care assessments and various medically related tasks.

Eligibility- 1 year of experience as a Visiting LVN for Home Health (Required)
- 1 year of nursing experience (Required)
- Current CA LVN License (Required)
- BLS certification from the American Heart Association and/or the American Red Cross (Required)
- NSO Nursing License Insurance (Required)
- IV/Phlebotomy Certified (Preferred)
Work at least 2‑3 days per week (or ten productivity units), one weekend per month, and two holidays per year (one winter, one summer).
Responsibilities- Provide nurse visits to home health patients.
- Offer care for ill, injured, or convalescing patients by implementing the plan of care initiated by an RN.
- Give basic patient care and treatment.
- Document consistently with the plan of care.
- Communicate with patients to schedule visits.
- Educate patients on self‑care during non‑visit days.
- Demonstrate knowledge of regulatory requirements.
On the road, providing care within a cluster of patients in the city closest to you.
