Licensed Vocational Nurse (LVN) - Full-Time

Golden Haven Care Center, a 99-bed Skilled Nursing Facility in Glendale, CA, is seeking a compassionate and dedicated Licensed Vocational Nurse (LVN) to join our team. Under the supervision of a Registered Nurse (RN), the LVN is responsible for providing quality patient care, applying the nursing process, and supporting the overall well-being of our residents.

Position Summary

The Licensed Vocational Nurse (LVN) is responsible for delivering high-quality nursing care in accordance with established standards and facility policies. The LVN works collaboratively with the interdisciplinary team to assess, implement, and evaluate patient care while promoting a safe and compassionate environment for residents.

Schedule

• Full-Time

• Rotating 4 days on / 2 days off schedule

• Work performed at a single location in Glendale, CA

Available Shift

• Flex Schedule: 2 days AM, 2 days PM

  • AM Shift: 7:00AM - 3:00PM

  • PM Shift: 3:00 PM – 11:00 PM

Pay

• $38.00–$43.00 per hour, based on experience

Responsibilities

• Utilize the nursing process to assess, plan, implement, and evaluate patient care

• Conduct focused patient assessments to identify individual care needs

• Monitor and assess residents for physiological and psychosocial changes, reporting significant findings promptly

• Collect, analyze, and document patient information and nursing observations accurately

• Administer medications safely and in accordance with physician orders and facility policies

• Maintain complete and accurate medical documentation

• Collaborate with physicians, Registered Nurses, and other healthcare professionals to ensure quality patient outcomes

• Perform additional duties as assigned

Qualifications

• Current California Licensed Vocational Nurse (LVN) license (required)

• Current BLS Certification (required)

• Minimum 1 year of LVN experience in a skilled nursing or related setting preferred

• Ability to communicate effectively in English

• Authorized to work in the United States
