Job DescriptionThe ICMS Supervisor provides leadership and oversight to Case Managers within the Intensive Case Management Services (ICMS) Program. Working in partnership with the ICMS Program Manager, the ICMS Supervisor ensures the delivery of high-quality, client-centered intensive supportive services to individuals residing in permanent supportive housing through Housing for Health (HFH).
Clients are referred through Homeless Services and Housing (HSH) and meet the HUD definition of chronically homeless. Many have significant service needs and are high utilizers of Housing for Health services. In addition to maintaining a client caseload, the ICMS Supervisor assists with staff supervision, caseload monitoring, quality assurance, training, documentation compliance, and program operations.
The ICMS Supervisor supports outreach and engagement, intake and assessment, service planning, housing stabilization, crisis intervention, eviction prevention, benefits establishment, and coordination of supportive services. This position also conducts quality assurance reviews, mentors staff, and helps ensure compliance with contractual and program requirements while promoting harm reduction and housing retention best practices.
Duties and Responsibilities Leadership & Team Oversight- Assist the ICMS Program Manager in supervising and supporting ICMS Case Managers.
- Monitor team caseloads and ensure staff maintain caseloads of 20–25 clients.
- Review and coordinate referrals from HSH and Housing for Health partners.
- Distribute referrals to ensure equitable caseload assignments.
- Ensure client intakes, assessments, and enrollment activities are completed within required time frames.
- Accompany Case Managers on client visits as needed to ensure staff safety and service quality.
- Assist with onboarding, training, coaching, and mentoring new staff.
- Facilitate case conferences and team meetings to support service delivery and client outcomes.
- Assist with client intake and enrollment activities.
- Support clients with obtaining eligibility documentation and completing enrollment requirements.
- Review client assessments and supporting documentation for completeness and accuracy.
- Ensure psychosocial assessments, care plans, and housing-related assessments are completed according to program requirements.
- Conduct monthly audits of client files and electronic documentation.
- Review CHAMP and HMIS records to ensure timely and accurate documentation.
- Monitor progress notes, care plans, assessments, budgets, and other required program documentation.
- Assist with maintaining active and exited client rosters for reporting and audit purposes.
- Develop and update program forms and tracking tools as needed.
- Support continuous quality improvement initiatives and program compliance efforts.
- Ensure client contacts meet ICMS Statement of Work requirements.
- Support staff in linking clients to medical, behavioral health, financial, housing, and community-based services.
- Provide guidance regarding crisis intervention, housing retention, and eviction prevention strategies.
- Promote harm reduction and housing stabilization best practices.
- Assist with implementing additional program requirements as directed by Housing for Health and County partners.
