Sangre de Cristo Community Care is looking for a Full Time Registered Nurse to join our dynamic hospice team in Walsenburg/ Trinidad, CO. Responsibilities:
Plans, coordinates, implements, and evaluates the nursing care of all patients.Utilizes clinical knowledge and skills to identify patient needs and coordinate services and referrals to meet these needs.Works closely with the interdisciplinary team to provide a continuous appropriate, comprehensive, and responsive plan of care for patients.Requirements:
