Colorado Veterans Community Living Center at Rifle
The Colorado State Veterans Community Living Center at Rifle is located on Colorado's western slope in the scenic Colorado River valley. Rifle offers a mild climate, with plenty of clean mountain air and blue skies. The facility provides 89 beds of skilled nursing care, including an 18-bed Special Care Unit for residents with Alzheimer's Disease or related conditions. The center serves veterans, their spouses, and eligible "Gold Star" parents, and is committed to caring for the whole person by addressing personal, social, physical, and medical needs.

Summary of PositionThis position exists to provide professional nursing services to facility residents, including assessment and specialized nursing care consistent with professional standards and applicable regulatory requirements.
Duties of the Position- Standard Assessments – Performs admission and daily assessments during clinical stays, identifies critical incidents, emergencies, acute illnesses, and behavior changes; develops and implements appropriate nursing interventions, treatment care plans, and physician referrals; documents care and treatment in Electronic Health Records and paper records; notes changes in status, medications, and physician orders; detects abnormalities in health status and reports to the physician.
May drive a state vehicle for off-site pre-admission assessments or transporting residents to appointments if not covered by ambulance. - Regulatory Compliance – Ensures nursing care complies with standards, policies, procedures, and regulatory requirements; directs subordinates to meet standards; reviews and audits physician orders, medication administration records, CNAs’ sheets, bathing schedules, and other documentation as part of quality assurance.
- Medication and Treatment Administration – Performs prescribed, specialized treatments and safe, accurate medication administration.
- Safety Identification and Intervention – Monitors resident safety, conducts rounds, implements clinical and environmental interventions, documents for regulatory compliance, and notifies family, physician, Nurse in Charge (NHA) and Director of Nursing (DON) as needed.
- Training, Resource Team Member and Delegation – Serves as a resource to staff, trains subordinates, ensures competencies, provides input on direct care performance, manages workload, prioritizes assignments, and delegates duties to ensure quality and continuity of care.
- Communication – Provides active listening, support, and communication with residents and families via phone and in person.
Education/Licensure/Certification/Experience
- Current, valid licensure as a Registered Nurse from the Colorado Board of Nursing or any state participating in the Enhanced Nurse Licensure Compact (eNLC) and two (2) years of professional experience in a field related to the position.
- Substitutions – Appropriate education may substitute for up to two (2) years of required experience on a year-for-year basis.
Special Qualifications
- This position must maintain a current CPR card and perform CPR in accordance with CPR Guidelines.
Conditions of Employment
- Must complete a full background check to include state and federal checks, licensure verification, drug screen, PREA screening, and other specified checks.
- Pre-employment physical and TB test may be required.
- Shift Work:
Day, evening, and night shifts to include holidays and weekends as needed. - Must possess a valid RN license in good standing. Colorado RN license or license in a participating state in the Nurse Licensure Compact is required to perform essential functions.
