Responsibilities:- Conduct initial and ongoing assessments of patients' nutritional needs and develop personalized meal plans
- Collaborate with healthcare team members to ensure comprehensive care and address any nutritional concerns
- Educate patients, families, and caregivers on proper nutrition and meal preparation techniques
- Provide nutrition counseling and support to promote healthy eating habits and disease management
- Develop and implement nutrition protocols and policies to ensure compliance with regulatory guidelines
- Monitor and evaluate the effectiveness of nutrition care and make recommendations for improvement
- Registered Dietitian (RD) certification or eligibility for RD certification
- Bachelor's or Master's degree in Nutrition or related field
- Experience in long-term care, acute care, or other healthcare settings
- Strong communication and interpersonal skills
- Ability to work effectively as part of a multidisciplinary team
- Familiarity with electronic medical records (EMRs) and nutrition software
