** Responsibilities:*
* * Provide direct patient care, including administering medications, performing wound care, and assisting with activities of daily living (ADLs)
* Develop and implement individualized care plans, in collaboration with the interdisciplinary team
* Monitor and report patient progress, including identifying and addressing any concerns or changes in condition
* Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination
* Maintain accurate and detailed records, including completion of patient charts and reports
* Participate in quality improvement initiatives and contribute to a culture of continuous learning and growth
** Requirements:*
* * Current LPN license in the state of Wisconsin
* Minimum of 1 year of experience in long-term care or post-acute setting
* Strong clinical skills and knowledge of nursing practices and standards
* Excellent communication and interpersonal skills
* Ability to work effectively in a fast-paced environment and prioritize tasks accordingly
* Compassionate and empathetic approach to patient care
