• Evaluate residents to determine OT needs and establish individualized treatment plans.
• Deliver direct therapeutic interventions addressing activities of daily living, mobility, cognition, motor skills, and safety in daily routines.
• Reassess progress regularly, document outcomes, and adjust goals and plans as needed.
• Collaborate with nursing, rehabilitation, and medical teams to coordinate care and support discharge or placement planning within the LTC setting.
• Educate residents and families on strategies, equipment, and environmental modifications to maximize independence and safety.

• Active Occupational Therapist license (state-specific) to practice in the United States.
• Availability for a 13-week, 36-hour-per-week assignment with daytime hours.
• Ability to work effectively in a long-term care setting and participate in a multidisciplinary team.
• Strong documentation, communication, and care coordination skills.
