• Consistent weekday schedule (M–F, 8–5)
• Shared on-call rotation (limited, typically holidays only)
• Defined coverage areas across Glenshaw, South Park, Sewickley, and nearby communities
• Team-based approach to care planning and patient support
• Clear expectations around documentation and time management

• Deliver hospice care to patients in their homes
• Collaborate with an interdisciplinary care team
• Manage your daily schedule and patient visits independently
• Complete charting within established timelines
• Utilize EMR and mobile tools to stay organized in the field

• A structured environment with established processes
• Support from leadership and peers when needed
• The ability to focus on patient care with clear expectations
• A more predictable workflow compared to less organized settings

• Active RN license in Pennsylvania
• 2+ years hospice experience
• Comfortable working independently in a field-based role
• Familiarity with EMR systems (Tru Chart or similar is a plus)
• Reliable transportation for local travel
