Registered Nurse – Nurse Care Coordinator

Job Summary:

The Nurse Care Coordinator is a healthcare professional responsible for managing both inpatient and outpatient care coordination to ensure seamless patient admissions and transitions. This role involves collaborating with healthcare teams to optimize patient outcomes and facilitate access to necessary services.

Responsibilities:

• Coordinate inpatient and outpatient services throughout the admission and treatment process.
• Assess patient needs and develop individualized care plans in collaboration with healthcare teams.
• Manage transitions of care, ensuring timely communication and follow-up.
• Educate patients and families on treatment plans and healthcare resources.
• Document patient interactions and care plans accurately in electronic medical records.

Qualifications:

• Hold a valid RN license issued by the state or relevant licensing authority.
• Graduate from an accredited nursing program recognized by the state board of nursing.
• Possess a minimum of 3 years of RN case management experience.
• Demonstrate proficiency with hospital electronic health records, including Epic.
• Exhibit strong communication and interpersonal skills.
