- Coordinate inpatient and outpatient processes within the admission and discharge workflow to optimize patient flow
- Assess patient needs and develop case management plans to support timely, appropriate discharge and resource utilization
- Collaborate with physicians, nurses, social workers, and external providers to coordinate seamless care transitions
- Facilitate access to services, referrals, and follow-up care; communicate barriers and progress to the care team
- Document care plans, interventions, and outcomes in the medical record; leverage health IT systems (Epic experience preferred)
- Ensure compliance with regulatory standards and maintain a patient-centered approach throughout the care trajectory
- Active Colorado RN license
- Graduation from an accredited RN program
- Minimum of 3 years RN Case Management experience
- Ability to work in a hospital-based setting and participate in rotating 12-hour shifts with weekend coverage
- Epic experience preferred; strong collaboration and communication skills
- Travel eligibility: permanent address must be at least 75 miles away from the assignment site
- Housing assistance or nearby housing availability for the assignment
