• Serve as primary point of contact for incoming and outgoing patient referrals. Triage referrals, gather necessary information, ensure timely processing and assignment to appropriate providers.
• Facilitate communication, collaboration, and coordination of care. Coordinate appointments, referrals, transitions of care between primary care, specialists, hospitals, and other healthcare settings, ensuring seamless transitions and continuity of care.
• Schedule patients utilizing coordinated provider list (CPL), make necessary arrangements related to the appointment, notify patients of appointment information: date, time, and location.
• Use web-based insurance platforms to generate referral authorizations.
• Effectively communicate the physicians/clinicians needs or outstanding items to patients.
• Follow all referrals through to completed appointment and obtain all documentation related to appointment, uploading into the organization’s medical record system for physician review prior to PCP follow-up appointment.
• Ensure any missed external appointments are rescheduled and communicated to the PCP.
• Address referral-related phone calls from patients, providers, etc. Complete and address phone messages in a timely manner.
• Provide extraordinary customer service to all internal and external customers.
• Perform other related duties as assigned.

$17.0 - $24.26 hourly. The posted pay range represents the base hourly rate or base annual full-time salary for this position. Final compensation will depend on a variety of factors including but not limited to experience, education, geographic location, and other relevant factors. This position may also be eligible for bonuses or commissions.
