LPN Licensed Practical Nurse

Heritage Health Care Center, part of HHP Lifeshare in Minster, OH, seeks a Licensed Practical Nurse to provide compassionate, person-centered nursing care for individuals with developmental disabilities in residential and community settings. The LPN administers medications and treatments, monitors health status, documents care, and collaborates with RNs, physicians, and direct support staff. This role includes health teaching, supporting daily living needs, and responding to changes in condition while promoting independence, dignity, safety, and quality of life in a collaborative, family-like team environment.

Responsibilities

• Provide person-centered nursing care to individuals with developmental disabilities
• Administer medications and treatments per provider orders
• Monitor, document, and report changes in health status
• Support daily living activities and promote independence and dignity
• Collaborate with RNs, physicians, therapists, and direct support staff
• Maintain accurate, timely clinical documentation and care records
• Participate in care planning and interdisciplinary team meetings
• Educate individuals and families on health, wellness, and medications
• Follow infection control, safety, and regulatory standards
• Support a compassionate, team-oriented, family-like environment
