Job Overview

Registered Nurse (RN) – Home Health.

Location:

Gloucester City, New Jersey. Provides care to patients in Philadelphia, PA and surrounding areas.

Hours:

40 per week (8‑hour shifts).

Employment type:

Staff. Ongoing position.

Responsibilities

As a home health registered nurse case manager, you will lead and collaborate to manage and deliver patient care plans. Your day‑to‑day duties include:

  • Traveling to patient locations (approximately 20% of your day).
  • Charting and documenting patient care and meeting all critical deadlines.
  • Self‑managing time and schedule to meet quality care standards.
  • Using tablets, mobile phones, and EMR software to support documentation and communication.
Qualifications
  • RN license in the state where you practice.
  • Graduate from an approved school of practical nursing.
  • One year of RN experience with clinical competence to deliver quality patient care.
  • Current driver’s license.
  • Commitment to meet charting deadlines.
  • Experience with tablets, mobile phones, and EMR software.
Compensation & Bonuses

Includes a first‑year success bonus of up to $10,000, shift premium pay, and other compensation based on caseload. Compensation potential varies by market.

Includes compensation related to patient care visits, non‑visit activities, paid days off, shift premium pay, and first‑year success bonus. The low end of the range is the average annual compensation earned based on a fully productive caseload; the high end reflects those who exceed productivity.
