Position Summary
The primary purpose of this position is to provide quality nursing care to residents with routine daily nursing care and services in accordance with the resident's assessment and care plan and as directed by the supervising nurse.
Responsibilities- Provides quality nursing care to residents in an environment that promotes their rights, dignity, freedom of choice, and individuality.
- Demonstrates respect, attention, and awareness toward the diversity among the people we serve.
- Is knowledgeable of the individualized care plan for residents and provides support in accordance with that plan.
- Identifies and participates in QAPI processes as directed.
- Assists in daily requirements and tasks in care and treatment of residents, including bathroom assistance, personal hygiene, dressing, mobility assistance, food intake, and other needs as required.
- Maintains resident rights and confidentiality.
- Documents all ADLs in the electronic health record.
- Observes and reports symptoms, reactions, and other changes in condition promptly.
- Routinely turns and positions residents as required by the care plan.
- Complies with resident rights and promptly reports incidents or evidence of abuse or violations to the charge nurse or administration.
- Honors residents’ right to refuse care and reports such refusal to the nursing supervisor.
- Assists in maintaining a safe, neat, and clean environment.
- Answers resident call bells promptly and courteously.
- Ensures that residents who cannot call for help are checked frequently.
- Takes and documents temperature, pulse, respirations, food and fluid intake and output, height and weight, and records them in the EHR.
- Ambulates and transfers residents using the appropriate assistive devices and body mechanics.
- Uses only the equipment trained on, operates safely, and reports defective equipment to the supervisor immediately.
- Communicates and interacts effectively and tactfully with residents, visitors, families, peers, and supervisors.
- Assists and escorts residents to appointments such as hair salon visits, activities, church services, or doctor appointments.
- Active MA certified nursing assistant license.

Salary Range: $19.00 – $24.00 per hour, based on experience.
