Primary​/Specialty Care Manager

TITLE:

Primary / Specialty Care Manager

SHIFT: 8:00AM - 4:30PM STATUS: 40 HOURS

The purpose of this job is to identify and provide care management services for the high-risk patients in the Primary/Specialty Care population and work with behavioral health agencies outside the Nathan Littauer Hospital Primary/Specialty Care system to coordinate PCP services for the Agency's high-risk population.

EDUCATION:

Possess a High School Diploma or equivalent.

PREFERRED:
Associate's degree with a concentration on care management or related discipline.

EXPERIENCE:

Care Management experience.

PREFERRED:
Process Improvement, Quality Improvement, and Patient Centered Medical Home recognition is highly preferred.

KNOWLEDGE:
Possess a working knowledge of outpatient, third party reimbursement, operations of a primary/specialty care private practice, managed care, computer operations.
