Position Summary:
We are seeking a dedicated and skilled Licensed Practical Nurse (LPN) to join our home care team. The ideal candidate will perform in-home patient visits, providing nursing care and education in collaboration with our interdisciplinary team.
Key Responsibilities:
Conduct home visits to provide skilled nursing care, including assessments, medication administration, wound care, and treatments.
Monitor and document patient conditions and progress.
Educate patients and families on managing medical conditions and promoting health.
Collaborate with the interdisciplinary team to ensure comprehensive and effective care plans.
Maintain accurate and timely electronic medical records (EMR).
Adhere to all safety protocols, infection control practices, and agency policies.
Qualifications:
Valid and active LPN license in New York.
Minimum of 1 year of nursing experience, preferably in home health, geriatrics, or acute care.
Current CPR certification.
Strong assessment, critical thinking, and communication skills.
Ability to work independently and travel within the service area (reliable transportation required).
Familiarity with EMR systems is a plus.
